Appointments & Visits

We are located in the Health Group Building, 4th floor, Suite 408 — directly behind the Kaiser building.
 Important: Entry is only available through the parking structure.

  • First visit: 60–90 minutes (includes evaluation and treatment).

  • Follow-up visits: Typically 45–60 minutes.

Wear comfortable clothing that allows easy access to the treatment area. Exercise clothing is recommended as treatment often includes movement-based rehab. Medical gowns are available if needed.

Yes. Parking is free. Enter the structure, turn right, and proceed to the second floor. Park there and enter the building directly.

  • Your insurance card
  • Medical prescription (if required by your insurance, e.g., Medicare)
  • Imaging reports (MRI, X-rays) if available

At Titan, we believe in a collaborative team model that ensures every patient receives high-quality, well-supervised care.

  • Physical Therapist (PT): The lead clinician responsible for evaluations, treatment planning, and overseeing all care. PTs often manage multiple patients and handle insurance/compliance documentation.

  • Physical Therapist Assistant (PTA): Works under the PT’s direction, usually with more one-on-one patient time. PTAs focus on guided exercises, manual therapy, and progress monitoring, enhancing the patient experience and outcomes.

Physical Therapy Technician (Tech): Provides support with non-clinical tasks such as equipment setup, exercise guidance, and maintaining the treatment area. While they do not provide hands-on clinical care, they streamline operations so PTs and PTAs can maximize effectiveness.

Insurance & Billing

Insurance coverage can be complex. Many carriers offer multiple plan types, making it difficult to confirm exact benefits until reviewed.

Please note:

  • We do not accept Medical (different from Medicare).
  • We do not accept HMO plans that restrict you to contracted clinics only.
  • We do not accept LA Care.

If we do not accept your plan, we offer discounted cash rates and can provide a superbill for possible reimbursement.

Yes. If you are receiving home physical therapy, you must be fully discharged from those services before starting outpatient PT at Titan. Medicare also requires a medical Doctor’s prescription/referral

Yes — we can assist with benefit review before your first visit. However, because plans can have multiple tiers and restrictions, we recommend calling your insurance provider directly for confirmation.

Yes, auto insurance claims are accepted. Lien cases are reviewed individually and taken on a case by case basis — please call for details.

We accept most major credit cards, checks, and cash for deductibles, co-insurances, and co-pays. Cash is accepted for all PT treatments and non-covered services.

  • Deductible: What you must pay out-of-pocket before insurance begins coverage.
  • Copay: A fixed fee per visit, determined by your plan.
  • Coinsurance: The percentage you are responsible for after meeting your deductible; insurance pays the rest.

We welcome self-pay patients. Discounted cash rates and superbills are available.

Yes. We are a preferred provider for most worker’s compensation insurances including Sedgwick, Medrisk, Align, and others.

Our fees are based on usual and customary rates for our area. We keep them competitive while maintaining above-standard service.
